I, William J. Cook, pursuant to 28 U.S.C. ¤1746, hereby
declare that:

ÿ1. I am a resident of the State of Illinois.
ÿðÿ
ÿ2. I own no property in the State of Texas and pay no taxes
there.
ÿðÿ
ÿ3. I derive no income from the State of Texas and conduct no
personal business there.
ÿðÿ
ÿ4. I have visited Texas briefly on approximately five
occasions: on a family vacation, while traveling with my father and
while driving through the State en route to another destination.
ÿðÿ
ÿ5. I was not present in Texas during any of the events giving
rise to this suit.
